Our Programs

School-Aged Health Education Paradigm

The Community Health Needs Assessment of Bergen County is a systematic approach to collect, analyze and use information to prioritize health issues and develop strategies to address them. The purpose of a CHNA is to understand the health status, behaviors, and needs of a particular community, and to identify factors that contribute to health disparities or inequalities. The assessment includes gathering data on health outcomes, behaviors, and determinants of health, such as socioeconomic factors, access to healthcare, and environmental factors.
